FOR INSURANCE PURPOSES

RELEASE AND WAIVER OF LIABILITY for LITTLE ORCHID TAROT

By booking a tarot reading on www.littleorchidtarot.com, I agree to the following terms of understanding:

I am a fully competent adult, 18 years or older, and voluntarily enter by my own free will into this agreement.

I release, waive, discharge, and covenant not to sue its sole proprietor, Karen O’Grady, any representatives, affiliates, proprietors, or agents (“the Business”) from all liability to myself, my representatives, assigns, heirs, next of kin, or representatives for any loss or damage to body, mind (i.e., alleged emotional or psychological), or property, whether in contract or in tort, that I may now have or in the future have against the Business, that is accidental, unintentional, or not caused by the Business, solely or contributorily.

I confirm that any information offered or explained is for entertainment purpose only and in no way constitutes legal, financial, or professional advice. I understand the Business will not diagnose illness or health conditions, including questions pertaining to pregnancy, paternity and death. I understand that there is no guarantee on the accuracy rate of a reading, session, or consultation.

Payment is due in full, no less than twenty-four (24) hours prior to my appointment. The Business assumes a right to cancel my appointment due to unpaid invoice(s). I understand that the reading will not commence until payment is received in full, and that once payment is confirmed by my financial institution, no cancellations or refunds will be accepted by the Business, up to and including as a result of my failing to appear at the confirmed time and date.

Should the Business find itself unable to perform the reading due to unplanned circumstances, illness or act of God, the Business will alert me by my preferred method of contact as soon as possible to reschedule the appointment.

I am fully aware and understand that by signing this legally enforceable document, I am waiving certain rights that I or my heirs, next of kin, executors, administrators, assigns, or representatives may have against the Business.

I am responsible and assume full risk for all my activities, my person, and my property during and following my reading with the Business.

I CONFIRM I HAVE READ, ACKNOWLEDGED, AGREED AND ACCEPTED THE TERMS OF THIS CONTRACT.
